To fail a welder qualification or a production inspection: a busted bend coupon opened past the limit, a busted shot showed rejectable indications. Most codes let a welder who busts a test retest, sometimes after documented training, but a pattern of busted shots gets people moved off critical work. It is used loosely for any rejected weld.
